This webinar will help health care leaders solidify their population and community health initiatives.
With a focus on tracking and measuring a patient's total health journey by addressing the social determinants of health, Taylor Justice discusses how to eliminate avoidable spending. Since 80 to 90 percent of a patient's health is determined outside of the health care system, this webinar will focus on how health care providers can digitally coordinate care with social services to prevent non-clinical ED visits and hospital readmissions. Specifically, the conversation will detail how to implement an effective population health model tactically and what pitfalls to avoid.
What You Will Learn:
- How to accurately define Population Health
- What are the goals of Population Health Initiatives?
- Population Health Models should not be manual; pitfalls to avoid
- Tactical and practical implementation models
Taylor Justice is a co-founder of Unite US and a U.S. Army Veteran. As a West Point graduate, Taylor was commissioned as a 2nd Lieutenant in the US Army as an Infantry Officer, and he received an honorable medical discharge from active duty in 2007. Like a lot of service members and their families, he lived through the difficulties of reintegrating into civilian life and was struck by the inefficiencies and confusion surrounding this process, which would play a critical role in his future venture. In 2013, he co-founded Unite US, with Dan Brillman and Andrew Price, a care coordination platform that helps extend the traditional healthcare provider network by creating a network of clinical providers and non-clinical social services. The proprietary platform allows all levels of the network to send and receive referrals, track patients, and share data creating a cohesive ecosystem that not only facilitates superior patient care but can help lessen some of the burdens on the system caused by high utilizers and the most vulnerable populations. The technology has expanded across the country and within a few short years has become the nation's leading health, human and social service referral platform. Prior to Unite US, Taylor co-founded HigherEchelon, Inc., with Dr. Joe Ross and Paul Maggiano, a human and organizational performance consulting firm with offerings in leadership, technology, engineering, and organizational development to public and private sector clients.
Taylor holds a BS from the United States Military Academy at West Point, and an MBA from Columbia University. He lives in New York City with his wife, Meghan, and their English Bulldog, Dude.
