Patient Handoffs
Each time a patient moves from caregiver to caregiver presents an opportunity for errors. Various tools and techniques can help streamline the handoff process and establish standardized communications between all staff members. Methods to reduce the chance of errors range from “I Pass the Baton” from the Department of Defense’s Patient Safety Program to the increasingly popular SBAR approach.
